IBC Forms (rDNA and Biosafety)
This page contains the forms required by the University to register all projects that involve the use of rDNA and/or biohazardous materials (including those whose protocols are deemed exempt by the NIH Guidelines). The IBC reviews and recognizes or approves any activity involving a) recombinant DNA, b) infectious agents capable of infecting humans, animals, or plants, c) select agents/toxins, and d) any other potentially biohazardous materials. Although federal regulations allow exemptions for some types of rDNA use, PI's must register all rDNA projects so that the IBC is aware of the activities and can verify/recognize that they are exempt.
To determine if your project is Exempt or Non Exempt, Form 1A the Exempt Registration Form asks six questions to help guide you to the correct form.
Form 1A: Exempt Recombinant DNA Registration Form
While it is important for all Principal Investigators to provide biosafety training specific to their lab to their lab personnel, Lab Biosafety #OH207 offered by EHS is highly recommended for labs conducting only Exempt rDNA studies. In addition, Principal Investigators and their lab personnel are not required to take RC301 or CW512 if the lab only conducts research that is Exempt from the NIH Guidelines.

Beginning in September 2007, researchers are asked to obtain approval for a biosafety protocol to accompany any Non Exempt rDNA protocol.
The biosafety protocol involves:
- Completion of Biosafety form
- Submission of an emergency plan
- Completion of a biosafety lab inspection (BSL-2 and BSL-3),
- Completion of two classes; Lab Biosafety course #OH207 and NIH Guidelines/UT IBC P&P course #RC301 or #CW512.
