Checklist for Final Submission of Dissertation or Treatise (fall, 2009)

You are required to upload your completed dissertation (see submission instructions) prior to submitting the following required paperwork to the Graduate School. All paperwork is due in this office no later than December 4, 2009.

• Committee Certification of Approved Version. This is the page printed from your dissertation which lists all of your committee members. It should be formatted as shown in Format Guidelines (.pdf). The printed hard copy must be signed by all members of your committee. All signatures must be original. No proxy signatures are permitted. This page does not have to be on cotton paper. Plain white paper is acceptable. Note: This page in the .pdf file should not be scanned in, therefore it will not be signed in the electronic file. Only the hardcopy will be signed.
  
  
• Additional Title Page. The date should be December 2009.
  
  
• Publication by UMI (.pdf). This confirms you have understood by submitting to UMI that you agree UMI is your publisher. All dissertations must be published; this is a requirement for the degree. (This form is not required for students in Liberal Arts and Natural Sciences. Students in these colleges who select UMI services must complete the UMI Submission packet [.pdf] available at the Graduate School forms page.)
  
  
• Copyright Disclaimer (.pdf). Shows you have permission to use any previously copyrighted material in your dissertation. Even if you did not use copyrighted material you do need to turn in this form. (This form is not required for students in Liberal Arts and Natural Sciences. Students in these colleges provide this disclaimer as part of their online submission to Texas Digital Libraries.)
  
  
• Doctor of Audiology Final Certification of Degree Requirements form to be completed by all Doctor of Audiology (AuD) Degree Candidates
  
  
• Survey of Earned Doctorates.  We hope that you will choose to take this survey, to help the National Research Council and this university keep track of the situation of doctoral graduates.  This form may be completed online at http://survey.norc.uchicago.edu/doctorate/index.jsp.
  
  
• Abstract and Vita. Submit one copy of your Abstract and one copy of your Vita printed directly from the .pdf file. These pages should be in the final version and format, not earlier versions.
  
  
• Intellectual Property Tutorial Certification. If you have not previously submitted the email verifying you successfully completed the tutorial, do so at this time.
  
  
• Statement on Research with Human Participants (.pdf). Submit this form whether you used human participants or not. If you did use human participants you need to attach a copy of the IRB approval letter or waiver or exemption notification to the form. If you previously submitted this paperwork to our office it is not necessary to do so again.
  
  
• Dissertation Publishing Agreement for The University of Texas General Libraries (.pdf). (This form is not required for students Liberal Arts or Natural Sciences.)
  
  
• Requests to Delay Publication. All dissertations produced at UT Austin are made available, open access, through Texas Digital Libraries. Students should note that restrictions and/or holds on publishing requested from UMI do not apply to the dissertation archived at the Texas Digital Libraries. A student may request permission from the Graduate Dean to delay making the dissertation available to the public through the Texas Digital Libraries for up to one year in order to protect patent or other rights. This request must be supported by a written recommendation from the dissertation supervisor and must be submitted and approved prior to your graduation. If no petition is made to the Dean, the dissertation will be searchable on the web and available for free from UT.
  
  
• Publication/Processing Fees. Students uploading to UMI are required to pay the associated publication and copyright registration (optional) fees at the time of upload. Students who select UMI Traditional Publication will be charged $65. Students who select UMI Open Access Publication will be charged $160. Students who select UMI Registration of Copyright services will be charged $65.
  
  Students uploading to Texas Digital Libraries are provided with Open Access Publication free of charge.
  
  All students are charged an $8 Graduate School dissertation processing fee. The billing for this fee is done through The University of Texas, and you will receive an email notification when this bill has been created. Payment of the dissertation processing fee may be made to the university either online or by check paid to the Cashier's Office in the Main Building, Room 8.

This paperwork may be submitted in person by December 4, 2009, by coming to the Graduate School, Main Building 101, or it may be mailed to arrive no later than December 4, 2009 (mail to: The University of Texas at Austin, VP & Dean of Graduate Studies, 1 University Station G0400, Austin, Texas 78712-0531).
