Doctoral, Master's and Other Forms
The following is a master list of links to forms you may need access to while enrolled as a graduate student at The University of Texas at Austin. If you don't find a form listed here it is possible that the policy requires submitting a petition or letter with required signatures. - Candidacy
- Candidacy Application - completed online in UT Direct. Your department will notify you when you are eligible to apply for candidacy.

- UMI Submission Packet (Publishing Agreement) - Required if you uploaded your dissertation to Texas Digital Libraries (TDL) and selected optional UMI services.
- Doctoral Committees
- Online Doctoral Degree Candidate Form - Required for filing to graduate. Complete by published deadline during the semester you plan to graduate.
- Leave of Absence
- Petition for Leave of Absence for doctoral students in candidacy
- Authorization for Leave of Absence for doctoral students not in candidacy
- Oral Examinations
- Request for the Final Oral (required 2 weeks prior to the defense)

- Program of Work - Beginning 2008, an electronic Program of Work that has been approved by the Graduate Adviser is a graduation requirement for all Master's Degree Candidates. This paper form is for use by students enrolled in 698A or by special permission of the Graduate Dean. Dual-degree students must use the Dual-Degree Program of Work.
