Scholastic Dismissal Appeal Form
Note: This form is intended for use by Liberal Arts students only.
Please review the instructions and information included on each form before proceeding. It is highly recommended that you meet with an academic advisor regarding your appeal or petition.
If you have received notification that you have been dismissed from the University of Texas at Austin and College of Liberal Arts and want to return before your full dismissal period is complete, you must submit a scholastic dismissal appeal form.
This form can also be used if you feel you were wrongly dismissed and would like to appeal the decision immediately.

All appeals and petitions must be turned into the Liberal Arts Student Division office (GEB 2.200) by the deadline specified each semester. Call (512) 471-4271 or check the College web for dates and deadlines. This form must be downloaded, filled out and submitted to:
